Adams County Connections will accept mentee referrals from all middle schools in Adams County School District 50—Hodgkins Middle School, Scott Carpenter Middle School, Clear Lake Middle School and Shaw Heights Middle School.

All referring faculty and staff must complete a referral form. Youth Referral Form

Once you have completed the form, you  submit it electronically to mentor@peerassist.org or accmentoring@gmail.com, 
or fax it to 303.369.0982 atten: Meagan Brncick, Mentoring Program Coordinator

or mail a hard copy to:
